By electronically signing below, I agree that:
I understand that Columbus Babywearing (hereafter “CBW”), its leaders, and/or its meeting/event participants (hereafter "members") are not professionals or experts and that I, as the caregiver, take full responsibility to use the carrier(s) properly. I understand that with all the benefits to babywearing, there are also risks to myself and the child I am wearing. I understand it is my responsibility to ensure the proper use of any carrier to keep myself and the child within my care safe.
I understand that if I have concerns I need to contact the carrier vendor or distributor directly.
I agree to inspect the carrier and to look for any flaws to determine if the carrier should not be used.
I waive any claim and agree that I will hold CBW, its leaders, and/or its members, harmless of and from any loss or damages or cause of action whatsoever arising from any injury to myself or my child related to or caused in any way by babywearing.
I agree that I will not use this carrier except in the way I have been instructed by CBW leaders.
I understand that carriers are due back at the beginning of the meeting so they can be used during the meeting, and that is my responsibility to ensure carriers are returned in a timely manner. Carriers returned after the first 30 minutes of the published meeting start time will be subject to a $5 late fee.
If a carrier is not returned by the end of its scheduled meeting, the individual will be responsible for a $20 late fee (per carrier upon the missed return window) in addition to the $10 monthly checkout fee (per carrier). The carrier will be due back at the same meeting the following month (next meeting on that day of the week, same location or as I've been advised today). Fees are due upon their due date and will assessed at that time, or upon their return and inspection of the carrier(s). Charges will be made against a credit card on file or with the “deposit check.” Repeat offenses from any of the CBW policies will result in a ban from borrowing lending library items.
I acknowledge that suck pads are available to rent at no charge to me with any CBW soft structured carrier or meh dai. Suck pads need to be washed, returned clean and dry. CBW does not have a specific detergent or other cleaning requirements for washing suck pads. If I do not return the suck pads at the time of carrier return, I understand that I will be charged a $15 replacement fee. Additionally, I acknowledge that once I have been assessed the replacement fee, I will not be reimbursed this cost regardless of whether the suck pads are returned at a later date.
I agree to return the carrier in the same condition it was in when I borrowed it from the lending library (hereafter “original condition”).
I understand that I can only check out the same carrier twice in a row.
I understand that I am not to wash the carrier I am borrowing, aside from spot cleaning, without first consulting a CBW Leader to obtain proper washing instructions. If I do wash the carrier, I promise to abide by the provided instructions and use the detergent instructed by the CBW Leader.
I agree that I will not check out a carrier if my residence is currently experiencing any pest problems including but not limited to bedbugs and fleas. If I discover any of these situations while borrowing a carrier, I will reach out to CBW Leaders to make arrangements for the return and cleaning of the carrier. I understand that I may be assessed additional cleaning and carrier inspection fees as required. Additionally, if I discover that I and/or the carriers in my possession have been exposed to any pest problems – including but not limited to bedbugs, scabies, or fleas – I will report the situation to CBW Leaders to make arrangements for the return and cleaning of the carriers.
Should I NOT adhere to either of these rules, my CBW membership will be terminated and I will be charged all inspection fees, cleaning fees, and any other expenses resulting from my failure to adhere to the aforementioned rules.
I agree that I will not check out a carrier if my residence is currently experiencing any high contagious communicable disease. If any carriers are [potentially] exposed to such, I agree to contact a leader to establish an adequate sanitation plan. Highly contagious communicable diseases can live on material for days, these include but not limited to chicken pox, measles, hand foot and mouth, etc.
I understand that I may not check out a carrier if I am a smoker or if the carrier will be exposed to an individual who is a smoker. I understand that if I return a carrier smelling of any strong or harmful odors or stains that cannot be sufficiently spot cleaned, I will be charged a variable fee depending on the amount of cleaning required to restore the carrier, up to the cost of replacing it.
If any carrier I borrow cannot be returned to its original condition prior to my checkout without compromising the integrity or life of the carrier, I acknowledge that a replacement fee will be charged. I understand that my deposit check will be retained until the situation has been fully addressed and the carrier will be left in checked-out status (no additional late fees will be charged)
If I elect to use a credit card for my deposit and checkout fees, I release CBW and its' leaders of any liability stemming from the processing of my credit card by Stripe.com. I understand the checkout will be processed using public Wi-Fi and accept any risks associated with the credit card transaction being completed. Furthermore, I acknowledge that my credit card information will be stored on file by Stripe.com for the duration of the checkout period in lieu of a deposit check. I understand that any expenses arising as a result of my use of the carrier will be charged to my stored credit card. These expenses include but are not limited to late fees, cleaning fees, damaged carrier, and replacement fees. Replacement fees up to $150 per carrier.
I understand that due to CBW allowing the flexibility of rolling over a deposit check from month to month that, should it become necessary to do so, an older check that I provided for such a purpose may be deposited at any time. If my check should be returned, I acknowledge that it will be my responsibility to promptly reimburse CBW for the amount of the original check as well as any insufficient fund or reversed check fees assessed by CBW’s financial institution. Should I have multiple instances of returned checks, I understand that I may lose the privilege of continuing to utilize checks as a payment and deposit method and/or may be banned from borrowing lending library items. Repeat offenses from any of the CBW policies will result in a ban from borrowing lending library items.
